双侧原发性乳腺癌25例临床分析

目的探讨双侧原发性乳腺癌的临床特点、诊治方法及预后。方法对25例确诊为双侧原发性乳腺癌患者的临床资料进行回顾性分析和总结。结果 25例原发性双侧乳腺癌患者中,同时性双侧乳腺癌4例,异时性双侧乳腺癌21例。病理类型：浸润性小叶癌1例,1例一侧为浸润性微乳头状癌并且伴有卵巢癌术后状态,一侧为浸润性导管癌,余23例为浸润性导管癌或导管内癌。25例均获随访,随访时间6个月-6年,死亡3例,均为双侧同时性乳腺癌,首发癌年龄均小于50岁。存活22例,经综合治疗后生存情况良好。结论双侧原发性乳腺癌经综合治疗后,疗效与单侧乳腺癌相似,预后主要取决于病理类型、分期及其生物学行为。第二癌的早期诊断及治疗是关键。

Clinical Analysis of 25 Cases with Bilateral Primary Breast Cancer

Objective To investigate the characteristics, treatment, prognosis of bilateral primary breast cancer. Methods A retrospective study was undertaken to analyze the clinical characteristics of 25 cases. Results In 25 cases,there were 21 cases of metachronous bilateral primary breast cancer and 4 cases of synchronous bilateral primary breast cancer. Pathological types of 25 cases were mostly invasire ductal carcinoma or intraductal carcinoma. 25 cases have been followed up, with the time from 6 months to 6 years, 3 cases of bilateral breast cancer died, with the starting age less than 50 years old. 22 cases survived in good condition after integrated treatment. Conclusion Bilateral primary breast cancer has the same curative effect as unilateral breast cancer. The prognosis depends on pathological staging and biological behavior. The key is early diagnosis and treatment for the cancer.
