自体脂肪颗粒注射移植的临床探究

目的:探讨自体脂肪颗粒注射填充移植的临床操作方法、适应证、操作技巧及并发症防治.方法:注射器手动低压注入肿胀麻醉液和注射器缓慢匀速吸脂,生理盐水中悬浮漂洗筛选脂肪颗粒,再用注射器在水面轻松的将质量好的脂肪颗粒根据需要吸入注射器内,低压注射填充.结果:术后均无严重并发症发生,随访1～7年,移植的自体脂肪存活率根据受植床组织及血运状况基本稳定在20%～90%,形体综合改善效果明显,手感良好,无供区畸形.结论:①自体脂肪颗粒注射填充移植注射器负压吸脂应首选;②脂肪颗粒倾入生理盐水悬浮、漂洗、筛选,注射器内自然静置冲洗法损伤最低,优于低压离心法;③尽最大限度缩短从吸脂到注射移植全过程的时间和减少对脂肪颗粒机械及化学损伤是提高脂肪成活率的关键;④保留注射器内自然静置后上层过去弃之的含脂质(油脂)部分和脂肪组织悬液注射填充移植,可作为多种组织工程的种子细胞,具有非常重要的应用价值.

The study on autogenous fat transplantation by injection

Objective To study the indication,complication,procedure,and operative skill in 570 cases that were injected autogenous fat.Methods Under tumescent infiltration,take the fat out gently,wash the fat granule,and inject them into recipient area.Results Follow up all the patients obtained good body contour or local appear.No complications appear.Conclusion Syringe injection has an advantage over the other methods.It should be washed clearly,static separation and injected very gently.
