脂肪因子及游离脂肪酸与非酒精性脂肪肝相关性研究

目的研究游离脂肪酸(FFA)及脂肪因子瘦素(LP)、脂联素(ADPN)与非酒精性脂肪肝(NAFLD)的关系。方法检测100例NAFLD患者及100例健康对照者FFA、LP、ADPN及空腹血糖(FPG)、空腹胰岛素(FINS)等,比较两组间差异,并分析NAFLD患者FFA、LP、ADPN与胰岛素抵抗(IR)的相关性。结果 NAFLD组FFA、LP及胰岛素抵抗指数(HOMA-IR)高于对照组(P0.01);NAFLD组ADPN低于对照组(P0.01)。NAFLD组HOMA-IR与FFA、LP呈正相关(r=0.374,P0.01;r=0.297,P0.05);HOMA-IR与ADPN呈负相关(r=-0.218,P0.05)。结论 NAFLD患者存在明显IR,检测LP、ADPN及FFA对NAFLD病情评估及指导治疗有重要意义。

Correlation between adipokines,free fatty acids and nonalcoholic fatty liver

Objective To study the relationship of free fatty acid(FFA),lipoprotein leptin(LP),adiponectin(ADPN)and non-alcoholic fatty liver(NAFLD).Methods FFA,LP,ADPN,fasting plasma glucose(FPG)and fasting insulin(FINS)of 100 patients with NAFLD and 100 healthy controls were tested.The differences between the two groups were compared.The correlation between FFA,LP,ADPN and insulin resistance(IR)in patients with NAFLD was analyzed.Results The FFA,LP and HOMA-IR in NAFLD group were higher than those in control group(P<0.01).The ADPN in NAFLD group was lower than that in control group(P<0.01).HOMA-IR was positively correlated with FFA and LP in the NAFLD group(r=0.374,P<0.01;r=0.297,P<0.05).HOMA-IR was negatively correlated with ADPN(r=-0.218,P<0.05).Conclusion NAFLD patients have significant IR.Detection of LP,ADPN and FFA are important for the assessment of NAFLD disease and guidance treatment.
