经电子支气管镜肺活检在肺部外围性病变中的应用价值

目的:探讨经电子支气管镜肺活检对肺部外围性病变的应用价值。方法:对356例肺部外围性病变的患者接受电子支气管镜肺活检、支气管肺泡灌洗、刷检,对细菌学、细胞学及病理学检查结果结合临床进行了回顾性分析。结果:本组356例,确诊阳性率为91.29%(325/356),其中肺癌为47.47%(169/356例),肺部炎性病变为27.94%(102/356例),肺结核为10.67%(38/356例),特发性间质性肺病为3.65%(13/356例),淋巴瘤为0.56%(2/356例),炎性假瘤为0.28%(1/356例)。阴性率为8.71%(31/356)。术后中小量咯血12例,占3.29%,发热7例,占1.97%,支气管痉挛3例,占0.84%,气胸2例,占0.56%,并发症发生率为6.74%,无1例死亡病人。结论:经电子支气管镜肺活检是诊断肺癌、肺结核及特发性间质性肺病的重要手段,对肺部外围性病变有较高诊断价值。

Application value of biopsy of lung by electronic bronchoscope to peripheral lesion

Objective ： To discuss the application value of biopsy of lung by electronic bronchoscope to peripheral lesion. Methods： This paper was retrospective analysis of bacterial, cytologic and pathological results of 356 cases of peripheral lesion, who received biopsy of lung, alveolar lavage and brush biopsy. Results： The positive rate was 91.29% （325/356）, of which lung cancer was 47.47% （169/356）, pneumonia was 27. 94% （102/356）, pulmonary tuberculosis was 10.67% （38/356）, idiopathic pulmonary fibrosis was 3.65%（13/ 356）, lymphoma was 0.56% （2/356）,inflammatory pseudotumor was 0.28% （1/356）. The negative rate was 8. 71% （31/356）. 12 cases had little to moderate hemoptysis in or after operation （3.29 % ）, 7 cases had fever （1. 97%）, 3 cases had bronchospasm （0. 84%）, 2 cases had peneumatothorax （0. 56%）. The incidence rate of complication was 6.74% （24/356） ,and no patient died. Conclusion .. Biopsy of lung by electronic bronchoscope to the cases of peripheral lesion is the major method to diagnose lung cancer ,pulmonary tuberculosis and idiopathic pulmonary fibrosis. And it is also the major differential diagnosis of pneumonic disease and other diseases. So it is a safe and effective method. And it has high application value of peripheral lesion of lung.
