侧入式两指快速分离法腹膜外剖宫产术140例临床分析

目的探讨侧入式两指快速分离法腹膜外剖宫产术的临床价值.方法将我院2003年6月至2004年12月间住院行侧入式两指快速分离法腹膜外剖宫产的产妇140例与同期行传统的子宫下段剖宫产术的产妇150例进行比较,对比观察两组手术效果及手术并发症等.结果两组剖宫产术式手术时间有显著性差异(P<0.05),切皮至胎儿娩出时间无显著性差异(P>0.05),术后胃肠功能恢复情况及术后病率有显著性差异性(P<0.05),而新生儿Apgar评分及出血量无明显差异(P>0.05).结论侧入式两指快速分离法腹膜外剖宫产术安全性高,创伤小、操作简便、手术时间短、术后恢复快.

Clinical analysis of 140 cases of extraperitoneal cesarean section modified by two-finger sideway separation

Objective To investigate the advantage of extraperitoneal cesarean section modified by two-finger sideway separation. Methods A total of 140 pregnant women who had indications for cesarean section underwent improved extraperitoneal cesarean section from June 2003 to December 2004, and 150 pregnant women underwent traditional lower uterine segmental cesarean section at the same period. The overall time of operation, amount of blood loss, difficulties of taking out fetal head, the postoperative period of breaking wind, and postoperative morbidity of the two groups were compared. Results As compared with the intraperitoneal cesarean section group, the lower uterine segmental cesarean group had a much shorter overall time of operation (P<0.05), a shorter postoperative period of breaking wind (P<0.05), but no significant difference in the amount of blood loss, the duration from the skin incision and getting the fetus head out, and the Apgar score of the newborns (P>0.05). Conclusion Extraperitoneal cesarean section is a safe, fast, convenient and easy-to-master operation.
