主动脉夹层超声心动图诊断及与其他影像诊断的比较

目的:探讨经胸超声心动图(TTE)对主动脉夹层(AD)的诊断价值,并与磁共振影像(MRI)、CT、血管造影(DSA)等影像学的比较。方法:对59例确诊为AD的影像学资料列表对照分析,评价各种方法检出率和误诊率。结果:TTE、MRI、CT、DSA对AD诊断的检出率分别是76.1％、92％、90％、100％。TTE对AD各部位的检出率主动脉根部最高(8/8),胸主动脉最低(12/26),漏诊率达54％。结论:TTE对AD有较好的诊断价值,其诊断敏感性与MRI、CT接近。TTE对胸主动脉AD的漏诊率最高,应引起重视。

Comparison of Value of TTE with other Imaging Techniques in Diagnosing Aortic Dissection

Objective: To explore the diagnostic value of transthoracic echocardiagraphy(TTE) in aortic dissection and compare it with other imaging techniques. Methods: The imaging data of 59 patients with AD were analyzed to assess the detectable rate and misdiagnostic rate for them detected by all kinds of imaging techniques. Results.: Sensitive of TTE, MRI, CT, DSA were 76.1%, 92%, 90%, 100% respectively. The detectable rate of AD in root of aorta was the highest one(8/8)when them detected by TTE; thoracic aorta is the lowest one(12/26), misdiagnostic rate was 54%. Conclusion: TTE is one of the optimal imaging techniques in diagnosing aortic dissection. The sensitive of TTE in diagnostic aortic dissection is commonly with MRI and CT. It must be noticed that misdiagnostic rate of TTE is the highest one for thoracic aorta AD.
