无创正压通气在COPD急性呼吸衰竭中的应用

目的探讨无创正压通气治疗COPD并急性呼吸衰竭患者的疗效。方法选择40例COPD并急性呼吸衰竭患者,随机分为对照组和治疗组各20例次,两组均给予抗感染、祛痰、解痉、糖皮质激素及氧疗。治疗组加用无创正压通气。观察患者血气及气管插管率的差异。结果治疗组的血气分析改善明显优于对照组(P<0.01);治疗组的气管插管率低于对照组(P<0.01)。结论无创正压通气可改善COPD并急性呼吸衰竭患者的病情及减少气管插管率。

The use of atraumatic positive pressure ventilation in COPD with acute respiratory failure

Objective To discuss the curative effect of atraumatic positive pressure ventilation in patients of COPD with respiratory failure.Methods Forty patients of COPD with respiratory failure were randomly divided into two groups: contrast group (n=20) and experiment group (n=20). In the contrast group, anti-infective, expectorant, spasmolysis, glucocorticoid and oxygen therapy were used. In addition, atraumatic positive pressure ventilation was used in the experiment group. The differences of blood gases and rate of endotracheal intubation in the two groups were evaluated.Results In the experiment group, the improvement of blood gases was better than that of the contrast group (P<0.01), and the rate of endotracheal intubation was lower (P<0.01).Conclusion The use of atraumatic positive pressure ventilation in COPD with acute respiratory failure can improve the patient's condition and descrease the rate of endotracheal intubation.
