经阴道无张力尿道中段悬吊术治疗女性压力性尿失禁的临床观察(附184例报告)

目的 探讨经阴道无张力尿道中段悬吊术(TVT)治疗女性压力性尿失禁(SUI)的临床效果.方法 184例SUI患者应用TVT治疗,对手术效果、麻醉方式、并发症等进行探讨.结果 184例患者均有效,治愈率为96.2%,改善率为3.8%.局部浸润麻醉组,术后尿潴留发生率同硬膜外组相比明显降低,差异有显著性(P＜0.05).结论 TVT是安全有效治疗女性SUI的方法.局麻下手术创伤少,术后并发尿潴留少.TVT可与其他妇科手术同时操作.

Clinical effect of intra-vaginal slingplasty and tension-free vaginal tape on 184 women with stress urinary incontinence

Objective The aim of our research is to investigate the clinical effect of the TVT for women with stress urinary incontinence. Methods 184 women with stress urinary incontinence underwent the TVT. The results of operation, the approach of anaesthesia, complications of operation and changes of urodynamics were observed. Results The procedure showed effectiveness in all patients. The cure rate was 96.2% , improvement rate was 3.8%. The frequency of urinary retention in patients underwent local infiltration anesthesia was lower than those underwent epidural caudal block (P<0.05). Conclusions TVT is an effective and safe way for treating stress urinary incontinence. Under local infiltration anesthesia, the volume of hemorrhage is less and operation in wound is small, the recouery time is short and, the frequency of urinary retention is low. It can be conducted at the same time with other qynecological procedures.
