偏头痛精神障碍与血流动力学变化之间的关系

摘    要:目的:探讨偏头痛患者精神障碍与血流动力学改变的相互关系。方法:对40例偏头痛患者及30例正常人分别进行心理检测(SCL-90、SAS、SDS)和经颅多普勒超声(TCD)检测,分析偏头痛患者精神障碍与颅内动脉血流动力学改变及其相互关系。结果:偏头痛组精神障碍主要表现为抑郁、焦虑、恐惧、敌对,伴有躯体主诉的增多和人际关系紧张;血流动力学的改变表现为颅内动脉血流速度的增快。精神障碍与血流速度增快有关。结论:偏头痛患者存在精神障碍和血流动力学的改变,二者是偏头痛的临床特点,且精神活动的异常与颅内动脉血流速度有关。

Abstract:Objective To investigate the relationship between mental disorders and the change of hemodynamics in migraineurs.Methods The intracranial blood flow was determined by transcranial Doppler(TCD)and psychological condition was tested by Symptom Checklist 90(SCL-90),Self-rating Depression Scale(SDS) and Self-rating Anxiety Scale(SAS) in 30 patients with migraine and 40 normal controls in this hospita1.Results The incidences of depression,anxiety,panic disorder,interpersonal relationship and somatization,the mean blood flow velocity of bilateral middle cerebral arteries(MCA) and basal arteries(BA) increased significantly in migraineurs group(P<0.01) compared with control group.The incidence of mental disorders correlated positively with increase in mean blood flow velocity of intracranial arteries in migraineurs.Conclusions The mental disorders and the change of hemodynamics occurred in migraineurs,and both are the clinical characteristic of migraineurs.Moreover,the mental disorders is correlated with the blood flow velocity of intracranial arteries.
