NP和CAP方案治疗晚期NSCLC的对比研究

目的对比观察NP和CAP方案治疗晚期非小细胞肺癌的疗效、毒副作用.方法收集晚期非小细胞肺癌94例,以随机、对照的原则分为NP方案化疗组和CAP方案化疗组,对比观察两者间疗效、毒副作用、缓解期和生存期的差异.结果NP方案化疗组46例患者的有效率39.1%与CAP方案化疗组48例患者的有效率35.4%间无显著性差异(P＞0.05);但NP方案化疗组5月的中位缓解期和9月的中位生存期较CAP方案化疗组的4月和7月均有所延长;除具有一般化疗药物所共有的毒副作用外,NP方案对骨髓造血组织的抑制,特别是对粒细胞系和巨核细胞系较CAP方案为重,另外其周围神经炎和静脉炎的发生率也较高.结论NP和CAP方案都是治疗晚期NSCLC的较为有效的化疗方案,但NP方案化疗组的缓解期和生存期较长,优于CAP方案化疗组,毒副作用均,如辅以G-CSF等支持对症措施,NP方案有更好的临床应用价值.

COMPARATIVE STUDY OF NP AND CAP SCHEME IN ADVANCED STAGE NON-SMALL CELL LUNG CANCER

Objective:To contrast and observe the efficacy and toxicity of NP and CAP scheme to non small cell lung cancer.Methods:94 cases of advanced stage non small cell lung cancer were separated into two groups to contrast and observe the difference of the efficacy,toxicity,relievable and survival period according to random and contrast principle of statistics.One used NP scheme,the other used CAP scheme.Rusults:The effective rates of the two groups had no marked difference ( P >0.05).(The one of 46 cases used NP scheme is 39.1 %,the one of 48 cases used CAP scheme is 35.4 %).But the middle relievable period of five months and the middle survival period of nine months in the group used NP scheme were longer than the middle relievable period of four months and the middle survival period of seven months in CAP group.In toxicity,the two schemes had common toxicity as other chemotherapy drugs.But its effect of bone marrow restrain and its incidence of peripheral neuritis and phlebitis in the group used NP scheme were higher than the group used CAP scheme,especially affected the granulocyte and megakaryocyte line.Conclusion:NP and CAP scheme are all effective chemotherapy scheme to treat advanced stage non small lung cancer.But the relievable and survival period in the group used NP scheme are longer than the group used CAP scheme.The toxicity of the two groups is able to bear.If G CSF and supplementary drugs are used,the clinical value of NP scheme is better.
