99TcmO4-动态显像在颌下腺移位术中的临床应用

摘    要:目的探讨颌下腺动态显像在鼻咽癌颌下腺移位术中的临床价值。方法70例鼻咽癌初诊患者,分为试验组36例,非试验组(对照组)34例。对照组直接行常规放疗;试验组于术前1天行颌下腺^99Tc^mO4^-动态显像,根据结果选取功能较好一侧颌下腺于第2天行颌下腺移位术,术后复查颌下腺功能及择期行常规放疗。放疗后皆行颌下腺^99Tc^mO4^-动态显像及记录患者口腔干燥症状的程度,结果采用非参数秩和检验进行统计分析。结果移位侧颌下腺功能于术后、放疗后损伤明显减轻,与非移位侧颌下腺差异有显著性(P〈0.01);试验组中度以上口腔干燥症率为13.9%,对照组为76.5%,差异有显著性(P〈0.01)。结论^99Tc^mO4^-颌下腺动态显像可准确、灵敏地评估颌下腺功能。

Abstract:Objective To investigate the clinical value of 99TcmO4- scintigraphy in submandibular gland translocation in nasopharyngeal carcinoma ( NPC) patients. Methods Seventy NPC cases were divided into experimental group (36 cases) and control group ( 34 cases). The control group underwent routine radiotherapy directly. The experimental group was examined by radionuclide dynamic scintigraphy with 99TcmO4- 1 d before submandibular gland translocation. Based on the SPECT images, the better functioning submandibular gland was chosen to translocate next day. 99TcmO4- submandibular gland scintigraphy was repeated and radiotherapy started. The radiation induced xerostomia symptom of the patients was recorded. All data were statistically analyzed by Wilcoxon rank sum test. Results The radiation induced injury of translocated submandibular gland was significantly lowered. The functions of translocated submandibular gland and non-translocated one had significant difference after radiation (P <0. 01). Only 13.9% of the patients in the experimental group showed moderate or severe degree of xerostomia, whereas control group was 76. 5% , also showed a statistical difference between them ( P < 0. 01 ) . Conclusion 99TcmO4- dynamic imaging of submandibular gland can accurately and sensitively evaluate its function after translocation.
