寰枢椎肿瘤的根治性切除

目的:探讨寰枢椎肿瘤手术显露与根治性切除的可能性和方法, 以及枕颈内固定技术.方法:对19例经手术治疗的寰枢椎肿瘤病例进行回顾性分析.结果:19例中16例获平均32个月完整随访,其中12例未见肿瘤复发,1例术后死于呼吸道痰液梗阻,1例肿瘤复发放弃治疗,2例肿瘤复发后再次手术.17例伴有神经功能障碍者,Frankel分级平均提高1.6级.结论:寰枢椎肿瘤尽管治疗困难,但经合理的手术显露与根治性切除,仍可能获得良好疗效.

Atlantoaxial tumors treated with redical resection

Objective: To approach the possibility and techniques for atlantoaxial tumors exposure and radical resection. Methods: Nineteen patients with atlantoaxial tumors who underwent radical resection from April 1995 to December 2001 were studied and their clinical data were reviewed retrospectively. Results: 16 /19 cases were followed up for 7 to 79 months (32 months in average) effectively. There was no tumor recurrent evidence in 12 cases, but in 3 cases the tumors were recurrent (2 cases were operated again and the other patient refused operation). One patient died after operation due to acute trachea phlegm block. 17 cases with neurological disfunction obtained obvious improvement and their Frankel grade raised 1.6 degrees in average. Conclusion: Though atlantoaxial tumors were not easy to treat, the good clinical result could be acquired when they were treated by adequate exposure and radical resection.
