COX-2、E-cadherin和KAI1在乳腺浸润性导管癌中的表达及意义

摘    要:目的COX-2、E-cadherin(E-cad)和KAI1蛋白表达在乳腺浸润性导管癌(IDC)发生、发展中的作用及其与临床病理特征的关系。方法应用SP法检测82例IDC、20例乳腺导管内癌(DCIS)和20例乳腺单纯性增生距癌灶〉5cm的组织中COX-2、E-cad和KAI1蛋白的表达情况。结果IDC中,COX-2、E-cad和KAI1阳性表达率分别为87.8%、31.7%和47.5%,COX-2在IDC的阳性表达率高于DCIS和单纯性增生,E-cad和KAI1在IDC的阳性表达率低于DCIS和单纯性增生(P〈0.05),COX-2、E-cad和KAI1均与IDC患者年龄、肿瘤大小无关(P〉0.05)。COX-2高表达、E-cad和KAI1低表达与IDC组织分级、淋巴结转移呈正相关。COX-2表达与E-cad、KAI1表达呈负相关(P〈0.05)。结论COX-2高表达、E-cad和KAI1低表达可能是乳腺组织恶性转变以及IDC发生淋巴结转移的重要生物学标志,COX-2蛋白异常表达是IDC发生的早期事件,联合检测COX-2、E-cad和KAI1蛋白有助于判断IDC的生物学行为及预后。

Abstract:Objective To investigate the effects of COX-2,E-cadherin and KAI1 on the development and progression of invasive ductal carcinoma(IDC)and its relationship with the clinicopathological features.Methods Immunhistochemical S-P method was employed to detect the expression of COX-2,E-cadherin and KAI1 proteins in 82 cases of IDC,20 cases of ductal carcinoma in situ(DICS),and 20 cases of simple hyperplasia breast tissues that was >5 cm far from the tumor.Results The positive rates of COX-2 in IDC was 87.8%,which was higher than that in DICS and in simple hyperplasia breast tissues;The positive rates of E-cadherin,and KAI1 in IDC were 37.1% and 47.5%,which were lower than that in DICS and in simple hyperpllasia breast tissues(P<0.05).Three proteins were not related with age and site of tumor of patients(P>0.05),and there was positive correlation between over-expression of COX-2,decreased expression of E-cadherin and KAI1 with histological grade and lymph node metastasis of IDC.Conclusions The over-expression of COX-2,decreased expression of E-cadherin KAI1 might be important biological markers for malignant transformation in breast tissue and lymph node metastasis of IDC,and the abnormal expression of COX-2 is an early event of IDC.The combined detection of COX-2,E-cadherin,and KAI1 can help to predict the prognosis of patients.
