健康成人后交叉韧带MSCT与MRI对比研究

目的:探讨16层螺旋CT在健康成人膝关节后交叉韧带(PCL)成像中的价值及优势。方法:30侧健康成人膝关节行微体素CT扫描,其中左侧16例,右侧14例,其中20例行MRI检查。将CT扫描原始横断面图像数据传送至AW4.3工作站,分别进行多平面重建(MPR)及容积再现(VR)图像后处理,观察不同位置、不同角度的显示效果及CT表现,测量PCL的长径、厚度、CT值、PCL角、PCL指数等相关参数并与MRI测量值进行比较。结果:本组结果显示以膝伸直位正矢状面上显示PCL全长最佳,呈凸面向后、边缘光滑的弓形带状稍高密度影,各个指标的CT与MRI测量结果相比无统计学差异,部分测量值接近解剖测量值。结论:16层螺旋CT可以精确测量后交叉韧带及邻近骨结构,对指导临床应用及手术操作具有重要价值。

Comparative Study of 16-slice Spiral CT and MRI in the Evaluation of Posterior Cruciate Ligament in Healthy Adults

Objective:To study the significance and advantages of 16-slice spiral CT in the evaluation of posterior cruciate ligament (PCL) of knee joint in healthy adults.Methods:Thirty sides (16 sides in the left and 14 sides in the right) of knee joints in healthy adults were examined with micro-voxel CT scanning,20 sides of them underwent MRI simultaneously.All primary data of CT axial images were transferred to AW 4.3 workstation and post-processed with multiple planar reformation (MPR) and volume rendering (VR).The efficacy of demonstrating PCL from different positions and different angles were studied.The length,thickness,CT value,PCL angle,PCL index on CT images were measured respectively and compared with that of MRI.Results:The sagittal view of extending position was the best view in assessing PCL,which was showed as a little high density strip-like structure,stretching backforward like a bow.The data of CT and MRI had no statistical significant difference.Part of the results of CT measurements were similar to the anatomy findings.Conclusion:Accurate measurement results of PCL and adjacent bony structures could be obtained by 16-slice spiral CT,which was important in guiding clinical practice and surgery planning.
