摘    要:目的 探讨恶性血液病住院患者心理健康状态及与感恩、领悟社会支持的关系。 方法 采用非精神科住院患者心理状态量表(MSSNS)、领悟社会支持量表(PSSS)、感恩量表(GQ-6)对566例住院患者进行问卷调查,根据诊断疾病将患者分为恶性血液病组、其他恶性肿瘤组和非肿瘤组。 结果 恶性血液病与其他恶性肿瘤患者焦虑、抑郁、孤独得分均高于非肿瘤患者,差异有统计学意义(P<0.05),恶性血液病患者孤独得分高于其他恶性肿瘤患者,差异有统计学意义(P<0.05)。恶性血液病患者的焦虑抑郁愤怒孤独与感恩和领悟社会支持呈显著负相关(rs=-0.37~-0.50, P<0.001)。结构方程模型表明,领悟社会支持通过感恩间接影响恶性血液病患者的心理健康状态。 结论 恶性血液病患者较非肿瘤患者存在明显的焦虑、抑郁和孤独,较其他恶性肿瘤患者存在更为明显的孤独,感恩在领悟社会支持对心理健康状态的预测中起中介作用。

Abstract:Objective To investigate the mental health status and its relationship with gratitude and perceived social support in inpatients with hematological malignancies. Methods A total of 566 inpatients were surveyed with the Mental Status Scale in Non-psychiatric Setting(MSSNS), Perceived Social Support Scale(PSSS), and Gratitude Questionnaire-6(GQ-6). According to the diagnostic results, the subjects were divided into non-cancer group, hematologic malignancy group and other malignant tumor group. Results Compared with non-cancer group, the hematologic malignancy group and other malignant tumor group had higher level of anxiety, depression and loneliness(P<0.05). The hematologic malignancy group had higher score of loneliness than the other malignant tumor group(P<0.05). Gratitude and perceived social support were negatively correlated with anxiety, depression, anger and loneliness in the hematological malignancy group(rs=-0.37 - 0.50, P<0.001). The results of structural equation model showed that perceived social support indirectly affected mental health status and gratitude played a mediating role. Conclusion Compared with non-cancer patients, patients with hematological malignancies show obvious anxiety, depression and loneliness. Compared with patients with other malignant tumors, patients with hematological malignancies have higher level of loneliness. Gratitude acts as the mediator between perceived social support and mental health status.
