摘    要:随着对痛风研究的不断深入,2016—2017年发布了《英国风湿病学会(BSR)痛风管理指南》和《美国临床医师协会(ACP)临床实践指南:急性和复发性痛风管理》,包括对痛风的诊断、启动治疗的时机、急性期和慢性期的治疗方法、注意事项等。关于痛风诊断基因层面的研究逐渐增多,许多新的基因位点被发现;超声诊断对临床诊断痛风有一定的价值;降尿酸药物Lesinurad和Arhalofenate可以被用于治疗痛风。本文通过对指南进行解读,对痛风诊断和药物治疗的进展进行综述,以期为痛风治疗方法和药物研究提供理论支持。

Abstract:The study of gout is going deep and the guideline for the management of gout from the British Society for Rheumatology and the management of acute and recurrent gout:a clinical practice guideline from the American College of Physicians were published recently cover the essentials such as the diagnosis of gout,timing for the initiation of therapy,therapies for acute and chronic gout and precautions in the management.Moreover,updates of diagnosis of the genetic level of gout are embodied by newly found risk loci for gout and ultrasound becoming a promising new imaging modality for gout.Furthermore,two uric acid-lowering agents,Lesinurad and Arhalofenate,have been used for the treatment of gout.This article interprets the updated guidelines and reviews the recent developments in gout diagnosis and management,in order to provide a theoretical reference for clinical treatment and preclinical research.
