HPV分型检测联合TCT检查在宫颈病变筛查中的应用

目的 探究人乳头瘤病毒（HPV）以及液基细胞学（TCT）的联合检测在宫颈病变的筛查中的应用。方法 选取2015 年12月~2016年11月因宫颈异常来我院就诊的2327例妇科疾病患者，对其实施TCT以及HPV DNA检测，对阳性的患者进行阴道镜活检，以患者组织的病理学结果作为最终确诊标准。结果 共计355例患者进行阴道镜检查，其中HPV检测灵敏度为75.72%，准确性为70.43%，特异性为30.22%；TCT检测灵敏度为41.32%，准确性43.40%，特异性为47.87%；二者联合诊断灵敏度为88.21%，准确率75.10%，特异性为43.36%。两种检测方法的阳性检出敏感度相比，差异有统计学意义（P＜0.05）。结论 HPV分型检测联合TCT的检测方式具有相当高的阳性率及灵敏度，能够提高患者宫颈病变的检出率，延长患者的筛查间隔，有利于临床上对于宫颈病变的筛查。

Application of HPV Typing Test Combined with TCT Examination in Screening Cervical Lesions

Abstract:Objective To investigate the application of combined detection of human papillomavirus(HPV)and liquid-based cytology (TCT)in the screening of cervical lesions.Methods From December 2015 to November 2016,2327 patients with gynecological diseases who were diagnosed by cervical abnormality were examined by TCT and HPV DNA,and the positive patients were examined by colposcopy.Pathological findings of the patient's tissues were used as the final diagnostic criteria.Results A total of 355 patients were examined with colposcopy,in which the sensitivity of HPV was 75.72%,the accuracy was 70.43%,the specificity was 30.22%,the sensitivity of TCT was 41.32%,the accuracy was 43.40%,and the specificity was 47.87%;the sensitivity of the combined diagnosis was 88.21%,the accuracy rate was 75.10%,and the specificity was 43.36%.The sensitivity of the two detection methods was statistically significant(P<0.05).Conclusion The detection method of HPV typing combined with TCT has a high positive rate and sensitivity, which can improve the detection rate of the cervical lesions and prolong the screening interval of the patients,which is beneficial to the screening of cervical lesions.
