脐血、羊水促红细胞生成素含量与早产儿脑损伤的关系

摘    要:
目的: 探讨脐血及羊水中促红细胞生成素(erythropoietin,EPO)含量对早产儿脑损伤的早期预测价值。方法: 选取早产儿97例,根据头颅影像学检查结果分为脑损伤组和无脑损伤组(对照组),采用酶联免疫吸附法检测脐血及羊水EPO含量,分析脐血、羊水EPO含量与早产儿脑损伤的关系。结果: 脑损伤组早产儿脐血及羊水EPO含量均明显高于对照组(均P<0.05)。脐血EPO预测脑损伤的敏感性为83.8%,特异性为 78.3%;羊水EPO预测脑损伤的敏感性为75.7%,特异性为 83.2%。联合检测脐血及羊水EPO预测脑损伤的敏感性为75.7%,特异性为93.3%。 结论: 脐血及羊水EPO含量对早产儿脑损伤的早期预测具有一定价值。

Abstract:
Objective: To explore the predictive value of cord blood and amniotic fluid erythropoietin (EPO) in early brain injury in preterm infants. Methods: A total of 97 cases of premature infants were divided into brain injury group and control group according to the cranial imaging examination, the levels of EPO in cord blood and amniotic fluid were detected by enzyme linked immunosorbent assay (ELISA), then the relationship between premature brain injury and erythropoietin in cord blood and amniotic fluid was evaluated. Results: EPO levels of cord blood and amniotic fluid in brain injury group were significantly higher than those in the control group (both P<0.05).The sensitivity of cord blood EPO in predicting premature brain injury was 83.8% and the specificity was 78.3%.The sensitivity of amniotic fluid EPO in predicting premature brain injury was 75.7% and the specificity was 83.2%. The sensitivity of combined cord blood and amniotic fluid EPO in predicting brain injury was 75.7% and specificity was 93.3%. Conclusion: EPO may be used for the early prediction of brain injury in preterm infants.
