应变率弹性成像及剪切波弹性成像对TI-RADS 4类 甲状腺结节的诊断价值对比

目的 对比分析应变率弹性成像（SRE）及剪切波弹性成像（SWE）在TI-RADS 4类甲状腺结节中的应用价值。方法 对80例TI-RADS 4类甲状腺结节行细针活检穿刺，术前对结节均行SRE及SWE检查，根据病理结果对比分析两者对4类甲状腺结节的诊断价值。结果 80例结节中，经病理证实甲状腺乳头状癌46例，SRE敏感性及准确性分别为86.95%、59.70%；SWE敏感性及准确性分别为89.13%、64.06%，SRE联合SWE敏感性及准确性分别为89.74%、85.37%，SRE及SWE诊断准确性方面差异无统计学意义，SRE+SWE准确性高于单独应用SRE或SWE。结论 SWE对甲状腺4类结节诊断准确性略高于SRE，两者联合有助于提高4类甲状腺结节的准确性。

Comparison of the Diagnostic Value of Strain Rate Elastography and Shear Wave Elastography in TI-RADS Type 4 Thyroid Nodules

Abstract:Objective To compare the application value of strain rate elastography(SRE)and shear wave elastography(SWE)in TI-RADS type 4 thyroid nodules.Methods 80 patients with TI-RADS type 4 thyroid nodules underwent fine needle biopsy.SRE and SWE were performed on the nodules before operation.According to the pathological results,the diagnostic value of the two types of thyroid nodules was compared.Results In 80 cases of nodules,46 cases of papillary thyroid carcinoma were confirmed by pathology. The sensitivity and accuracy of SRE were 86.95% and 59.70%,respectively.The sensitivity and accuracy of SWE were 89.13% and 64.06%,respectively.The sensitivity and accuracy of SRE combined with SWE were 89.74% and 85.37%,respectively.There was no significant difference in the diagnostic accuracy between SRE and SWE.The accuracy of SRE+SWE was higher than that of SRE or SWE alone.Conclusion The diagnostic accuracy of SWE for thyroid nodules is slightly higher than that of SRE.The combination of the two can help improve the accuracy of the type 4 of thyroid nodules.
