保留悬雍垂扩大口咽气道治疗阻塞性睡眠呼吸暂停低通气综合征

目的 :评价保留悬雍垂 ,扩大口咽气道治疗阻塞性睡眠呼吸暂停低通气综合征的疗效。方法 :用改良悬雍垂腭咽成形术治疗阻塞性睡眠呼吸暂停低通气综合征 2 6例。结果 :术后随访 6～ 36个月 ,89%的患者主观症状明显改善 ,以AHI下降 >5 0 %为判定标准 ,有效率为 6 9.2 3%。结论 :此术式避免了过多切除软腭组织 ,有效地扩大了口咽气道 ,避免了腭咽关闭不全、鼻腔反流等并发症 ,提高了UPPP手术的疗效。

Elevated velum palatinum, preservated uvula and enlarged oropharyngeal air way in the treatment of obstructive sleep apnea and hypopnea syndrome

Objective:To estimate the clinical effect of the preserved uvula, elevated velum palatium and enlarged oropharyngeal air way in operation.Method: Twenty-six cases with obstructive sleep apnea and hypopnea syndrome(OSAHS) underwent the modified uvulopalatopharyngoplasty.Result: All patients were followed up 6 months to 36 months.89 percent of patients reported improvement of snoring and daytime sleepiness.69.2 percent of patients showed a decrease of at least 50 percent in the apnea and hypopnea indexes. No velum palatal insufficient or oro-nasal cavity backward flow was found after the operation. Conclusions: The results of the velum palatinum elevating, the uvula preserving and oropharyngeal air way enlarging UPPP indicate this procedure not only elevates the velum palatium, enlarge the space of the oropharynx effectively and improves the effect of the UPPP, but also avoids destroying its normal anatomical structure and physiological function and the postoperative complications.
