肺结节周围血管走行变化的CT观察

目的分析CT影像中肺结节周同血管走行变化征象的诊断价值。方法搜集经病理证实的80例肺结节病例，其中恶性结节39例，良性结节41例。所有病例行常规CT扫描，并对部分病例进行增强或对部分病灶进行减薄三维重建，观察结节周围血管走行变化征象。结果本组病例中周围血管分布改变的征象出现率为74％，良性结节为71％，恶性结节为77％，两者之间差异无显著意义（P〉0．05）。肺结节周围各型血管走行变化征象比较：Ⅰ型靠近，良性出现率为33％，恶性为20％，两者之间差异无显著意义（P〉0．05）；Ⅱ型穿过，良性出现率为22％、恶性为55％，两者之间差异有显著意义（P〈0．05）；Ⅲ型远离，良性出现率为16％，恶性为2％，两者之间差异有显著意义（P〈0．05）；Ⅳ型不变。结论肺结节周围血管走行改变征象有一定的规律性，识别这一征象对肺结节的定性诊断有重要意义。

Route Changes of the Peripheral Vessels around the Pulmonary Nodules with CT scanning

Objective To estimate the diagnosis value about route changes of the peripheral vessels around pulmonary nodules with CT. Methods Eighty patients with clinically and pathologically proved pulmonary nodules, in which 39 cases were malignant and 41, benign, underwent conventional CT scanning. Some patients were performed enhancement scanning or some images were reconstructed to view peripheral vascular route changing. Results The emerging rate of peripheral vascular distribution change was 74% （benign nodules were 71% and malignant nodules were 77% ） and there were no significant differences between these two types. The rate of nearby emergency among the benign nodules was 33% and that among malignant nodules, 20% , and there were no significant differences between them. The rate of which the vessels go through or far waay from the benign nodules was 22% , 16% , respectively, and those in malignant nodules, 55% anti 2% , respectively, and there were significant differences between them. Conclusion There were some regular patterns of the route changes of the peripheral vessels around these pulmonary nodules. There characters could help to differentiate diagnosis in pulmonary nodules.
