鼻内镜联合支撑喉镜在治疗声带良性病变中的应用

目的 探讨鼻内镜联合支撑喉镜在治疗声带良性病变中的特点和优势。方法 2004年1月～2006年1月，我科对100例声带良性病变患者进行该手术治疗，其中男76例，女24例，年龄28～79岁，平均54．3岁，临床表现均为声嘶。病理证实为双侧声带息肉57例，单侧声带息肉21例，单侧声带囊肿的4例，双侧声带囊肿6例，单侧声带内翻性乳头状瘤5例，双侧声带内翻性乳头状瘤2例，Reinker水肿5例。结果 所有患者都在全麻下鼻内镜联合支撑喉镜下使用显微喉钳完成手术，均完整切除病变，无手术并发症。术后随访12个月，失访7例，其余93例均未复发。结论 鼻内镜联合支撑喉镜在治疗声带良性病变中具有视野清晰、操作简便、精确度高、手术安全、术野暴露好等特点，尤其是对于声门暴露困难的病例更有其突出优点，值得临床应用推广。

Application of the treatment of benign vocal cord lesions by nasal endoscope combined with supporting laryngoscope

Objective To study the characteristics and advantages of the treatment of benign vocal cords lesions by nasal endoscope combined with supporting laryngoscope. Methods 100 patients were treated by nasal endoscope combined with supporting laryngoscope from January 2004 to January 2006. Among the patients, 76 patients were male while 24 patients are female. The ages of the patients were from 28 to 79 years, whose average age was 54.3 years. The clinical presentation of the patients was hoarseness. The pathological tested have proved that 57 cases were suffered from bilateral vocal polyps, and 21 cases suffered from unilateral vocal polyps, 4 cases suffered from unilateral vocal cysts, 6 cases suffered from bilateral vocal cysts, 5 cases suffered from unilateral vocal inverted papillomas, 2 cases suffered from bilateral vocal inverted papillomas, 5 cases suf- fered from Reinkeg edema. Results All patients were treated by nasal endoscope combined with supporting laryngoscope. The lesions were completely removed with no intraoperative complications left. The results of twelve months follow - up study showed that 93 cases were cured with no recurrence , and 3 cases lose of follow - up. Conclusion The method is simple, precise, safety, and also has characteristic of clear visual field and exposure. It is suitable for clinic application, especially for the patients suffered from bad exposure of the glottis.
