成人肠套叠的螺旋CT诊断

目的探讨成人肠套叠的病理基础及CT的诊断价值。方法回顾分析手术后病理证实的52例成人肠套叠的CT表现。结果 52例成人肠套叠的病因分别为:结肠癌33(63.5%)例,淋巴瘤4(7.7%)例,转移瘤1(1.9%)例,平滑肌瘤5(9.6%)例,脂肪瘤4(7.7%)例,结肠息肉3(5.8%)例,术后粘连2(3.8%)例。CT直接征象:靶征44例、肾形征7例、彗星尾征4例。CT间接征象:肠管壁增厚46例,肠梗阻43例。结论肠套叠的CT表现比较典型,其原发病变CT检出率高,部分病变可做定性诊断。因此,CT检查对成人肠套叠有重要的诊断价值。

Spiral CT diagnosis of adult intussusception

Objective: To explore the pathologic basis of intussusception in adults and the diagnostic value of CT.Methods: CT manifestation in 52 adult patients with intussusception were analyzed retrospectively.Results: The cases of 52 patients included 33 cases of colonic carcinoma,4 cases of lymphoma,1 case of metastatic tumor,5 cases of leiomyoma,4 cases of lipoma,3 cases of colon polypus and 2 cases of adhesion.CT direct signs were 44 cases of target sign,7 cases of kidney shape sign,4 cases of comet tail sign,and CT indirect signs,46 cases of thickening intestinal wall,43 cases of intestinal obstruction.Conclusion: CT can well display the intussusceptum,and has the higher detectable rate on the primary disease of intussusceptum,which is reliable for qualitatively diagnosing certain diseases.Therefore,CT has high value for diagnosing adult intussusception.
