对1例重症肺炎患者的药学监护

目的:探讨临床药师如何开展ICU患者的药学监护。方法:对1例ICU重症肺炎患者的初始及后续抗感染方案的制定、血液滤过时抗感染药物的调整、营养支持方案的制定,患者血糖的监测等方面,进行药物治疗监护,总结经验。结果:临床药师评估患者状况、建立用药监护计划、采取合适措施并跟踪治疗,避免了潜在药物治疗风险,解决了药物治疗存在的问题。结论:建立并完善用药监护要点,及时与医师沟通,充分发挥药学专业技能,有利于开展药学监护。

Pharmaceutical care for one patient with severe pneumonia

Objective: To explore pharmaceutical care for ICU patients by clinical pharmacists.Methods: Pharmaceutical care was given to one ICU patient with severe pneumonia in the aspects of selecting antibacterial drugs,adjusting drug dose during hemofiltration,applying nutrition support and monitoring the blood glucose.The process and experiences were recorded.Results: The clinical pharmacist evaluated the patient’s condition,drew up the plan of pharmaceutical care,selected appropriate measures and followed the treatment.The potential risk of drug therapy was avoided and the problem of drug therapy was solved.Conclusion: For carrying out pharmaceutical care,it is essential to set up the keypoints of pharmaceutical care,communicate with physicians in time and use professional pharmaceutical knowledge.
