胸腔镜下小切口纯钛肋骨接骨板固定术治疗多发性肋骨骨折的疗效分析

摘    要:目的:探讨胸腔镜下小切口纯钛合金肋骨接骨板固定术治疗多发性肋骨骨折的疗效。方法:抽取2015年1月至2016年7月收治的80例多发性肋骨骨折患者作为研究对象,根据患者意愿分为观察组(n=42)与对照组(n=38)。观察组行胸腔镜下小切口纯钛肋骨接骨板固定术,对照组接受非手术保守治疗。记录并比较两组患者治疗前、治疗后24 h血气分析水平、肺通气功能、疼痛视觉模拟评分、自主下床活动、住院时间、治疗有效率、骨折愈合率及并发症发生率。结果:治疗后,观察组患者血气分析水平、肺通气功能显著优于对照组(P<0.05)。观察组患者自主下床活动时间、住院时间、疼痛评分均显著低于对照组(P<0.05)。观察组患者治疗有效率为95.24%,骨折愈合率为100.00%,均显著高于对照组(76.32%与86.84%)(P<0.05)。观察组患者并发症发生率(26.19%)显著低于对照组(52.63%)(P<0.05)。结论:胸腔镜下小切口纯钛合金肋骨接骨板固定术治疗多发性肋骨骨折疗效显著,安全性高,值得临床推广。

Abstract:Objective:To investigate the effect of thoracoscopic-assisted small incision titanium rib plate fixation for the treatment of multiple rib fracture.Methods:Eighty cases of multiple rib fractures were selected as the research object between Jan.2015 and Jul.2016.According to the patient's voluntary choice of treatment,patients were divided into observation group and control group,which included 42 patients and 38 patients respectively.Observation group were given thoracoscopic-assisted small incision pure titanium rib plate fixation,and control group underwent conservative treatment.The two groups were observed and compared the level of 24 h blood gas analysis,pulmonary ventilation function,visual analogue scale (VAS),independent ambulation time,hospitalization time,efficient rate of treatment,fracture healing rate and complication rate before and after treatment.Results:After treatment,the levels of 24 h blood gas analysis and the levels of pulmonary ventilation function of observation group were significantly better than those of control group (P<0.05).The independent ambulation time,hospitalization time and VAS scores of observation group were significantly lower than those of control group (P<0.05).The effective rate of treatment (95.24%) and fracture healing rate (100%) of observation group were significantly higher than those of control group (76.32% and 86.84%,P<0.05).The complication rate of observation group was 26.19%,which was significantly lower than that of control group (52.63%,P<0.05).Conclusions:Thoracoscopic-assisted small incision pure titanium alloy rib plate fixation for the treatment of multiple rib fractures is significantly effective,highly safe and worthy of promotion.
