单孔胸腔镜手术治疗自发性气胸合并进行性血胸12例报告

目的:探讨自发性气胸合并进行性血胸的病因、诊断及治疗。方法:回顾分析2011年1月至2016年9月为12例自发性气胸合并进行性血胸患者行单孔胸腔镜手术的临床资料。结果:患者均于入院48 h内行单孔胸腔镜手术,手术均顺利,患者恢复良好,中位手术时间64 min,术后中位留管时间2 d,中位住院时间5 d。随访至今,12例患者均无复发。结论:自发性血气胸应采取个体化的治疗措施,对于入院时胸片或CT检查提示为中量及以上胸腔积液的患者,应更倾向于积极进行手术治疗;单孔胸腔镜手术更具微创优势,值得推广应用。

Single port thoracoscopic surgery in the treatment of spontaneous pneumothorax and progressive hemothorax:a report of 12 cases

Objective:To explore the etiology,diagnosis and management of spontaneous pneumothorax and progressive hemothorax.Methods:Twelve patients with spontaneous pneumothorax and progressive hemothorax underwent single port thoracoscopic surgery from Jan.2011 to Sep.2016,their clinical data were retrospectively analyzed.Results:All patients underwent successful single port thoracoscopic surgery in 48 h after admission and recovered smoothly.The median operation time was 64 min,the median postoperative drainage time was 2 d and the median postoperative hospital stay was 5 d.All patients were followed up till now without recurrence.Conclusions:Spontaneous hemopneumothorax is rare,potential life-threatening emergency,which is worthy to be focused on adequately.The treatment of the spontaneous hemopneumothorax should be individual.Positive operation should be recommended to the patients whose chest X-ray or CT examination prompts middle or large amount pleural effusion.Single port thoracoscopic surgery is more minimally invasive and deserves to be recommended in the clinical application.
