骶管药物注射治疗脊髓损伤后神经源性膀胱的疗效观察

【目的】探讨骶管内药物注射对脊髓损伤（spinal cord injury，scI）后膀胱功能障碍的疗效。【方法140例完全性SCI患者随机分为观察组（骶管内药物注射）和对照组（常规治疗），每组20例。对照组采用膀胱功能训练、间歇导尿、Crede手压法、心理干预。观察组在常规治疗的基础上联合骶管内药物注射。治疗前、治疗后分别评定患者日排尿次数、平均膀胱容量、最大排尿量、残余尿量、生存质量评分等。【结果】1个月的疗程结束后，患者日排尿次数、平均膀胱容量、最大排尿量、残余尿量，排尿症状和生存质量评分等指标较治疗前差异均有显著意义（P〈0．05），骶管内药物注射组明显优于常规治疗组（P〈0．05）。【结论】骶管内药物注射联合常规膀胱功能训练对治疗SCI患者神经源性膀胱的疗效优于常规的膀胱功能训练。

Observation on the therapeutic effects of sacral canal injection on neurogenic bladder in patients with spinal cord injury

[Objective ] To explore the effects of sacral canal injection on neurogenic bladder in patients with spinal cord injury （SCI）. [Method] Fourty patients with SCI were randomly divided into two groups： The observation group and conventional therapy group, twenty patients in each group. Conventional therapy group was treated with bladder function training, intermittent catheterization,, Crede maneuver or psychological intervention. The observation group was treated with sacral canal injection combined with conventional therapy. The effects of two groups were assessed with urination frequency, average bladder capacity, maximum urinary output, residual urine, quality of life before and after treatment. [ Result]There was significant difference of the urination frequency, the average bladder capacity, maximum urinary output, residual urine and quality of life in patients before and after treatment（ P 〈 0.05 ）. Curative effect of sacral canal injection group was significantly better than that of conventional therapy group（ P 〈 0.05 ）. [Conclusion ] Curative effect of sacral canal injection combined with conventional therapy was significantly better than that of conventional therapy for the neurogenic bladder in patients with SCI.
