高血压性脑出血预后相关影响因素研究

目的:探讨高血压性脑出血预后相关因素。方法:对128例高血压性脑出血患者的一般资料和临床资料进行调查,分析其预后特点及相关影响因素。结果:年龄、出血部位、出血量、出血是否破入脑室、有无中线移位、有无合并危险因素和发病诱因对高血压性脑出血的临床疗效均有影响(P<0.05~P<0.01)。logistic回归分析显示:出血部位、是否合并危险因素、出血是否破入脑室均对临床疗效有影响(P<0.01)。出血是否破入脑室、有无中线移位和发病到入院时间对出院时格拉斯哥评分有影响(P<0.05~P<0.01)。出血量、出血是否破入脑室、有无中线移位对高血压性脑出血患者的躯体综合功能评分有影响(P<0.05~P<0.01)。结论:高血压性脑出血临床疗效相关影响因素有出血部位、是否合并危险因素和出血是否破入脑室;但单独的神经功能评分和临床疗效的影响因素并不完全一致。

Relative factor affecting the prognosis of patients with hypertensive cerebral hemorrhage

Objective:To investigate the relative factors affecting the prognosis of hypertensive cerebral hemorrhage. Methods:The general data and clinical data of 128 patients with hypertensive cerebral hemorrhage were investigated, and the characteristics and influencing factors of prognosis were analyzed. Results:The age, bleeding site, bleeding volume, hemorrhage breaking into ventricle, midline shift,risk factors and predisposing factors affected the clinical efficacy of hypertensive cerebral hemorrhage( P<0. 05 to P<0. 01). Logistic regression analysis showed that the bleeding site,risk factor and hemorrhage breaking into ventricle affected the clinical efficacy(P<0. 05 to P <0. 01). The hemorrhage breaking into ventricle,midline shift and occurrence of disease to admission time affected the Glasgow Coma Scale score at discharge(P<0. 05 to P<0. 01). Bleeding volume,hemorrhage breaking into ventricle and midline shift affected the body comprehensive function evaluation of patients with hypertensive cerebral hemorrhage( P<0. 05 to P<0. 01). Conclusions:The bleeding site,risk factor and hemorrhage breaking into ventricle are the affecting factors of clinical efficacy in patients with hypertensive cerebral hemorrhage. The single neurological functional score and influencing factor of the clinical effect is not consistent.
