摘    要:目的:总结经尿道前列腺电切术(TURP)治疗老年良性前列腺增生(BPH)的围手术期处理方法。方法:回顾性分析69例施行TURP的老年BPH患者的临床资料,比较手术前后各项指标的差异。结果:69例均耐受TURP手术,并安全渡过围手术期,无病死病例。术中未发生大出血及经尿道前列腺电切综合征,术后早期未出现肺部感染及肺栓塞等严重并发症。所有患者术后12个月的国际前列腺症状评分、生活质量评分、最大尿流率及残余尿量与手术前差异均有统计学意义(P<0.01)。结论:术前针对老年BPH患者的高危因素进行个体化治疗,术中严密监测、规范操作、彻底止血以及术后完善细致的治疗是保证手术成功的关键所在。

Treatment of perioperative period in senile benign prostatic hyperplasia treated with transurethal resection of prostate

Abstract:Objective:To summarize the treatment of perioperative period in senile benign prostatic hyperplasia(BPH) treated with transurethal resection of prostate ( TURP ) . Methods:The clinical data of 69 BPH patients treated with TURP were retrospectively analyzed. The differences of the indexes in all cases between before and after operation were compared. Results:The 69 elderly patients withstanded the TURP operation and gotten safely through the perioperative period, and no death case was found. No bleeding and transurethral resection syndrome during the operation,and no pulmonary infection and pulmonary embolism in early after operation were found. The differences of the international prostate symptom score,quality of life score,maximum urinary flow rate and residual urine volume in all patients between before operation and after 12 months of operation were statistically significant(P<0. 01). Conclusions:The individualized treatment,strict monitoring,standard operation,thorough hemostasis,postoperative careful treatment in elderly BPH patients are the key in ensuring the success of operation.
