激光联合咪喹莫特乳膏治疗尖锐湿疣的初步研究

摘    要:目的:探讨激光治疗后继用5%咪喹莫特乳膏8周减少尖锐湿疣复发的疗效和安全性。方法:对64例尖锐湿疣患者进行前瞻性研究、其巾36例仅进行激光治疗(激尤治疗组),而另外28例在激光创面完全愈合后再外用5%咪喹莫特乳膏8周(联合治疗组)。观察患者在4周、8周和12周时的复发率及不良反应。结果:激光治疗组在4周、8周和12周时的复发率分别为25.0%、13.3%和3.7%,而联合治疗组相应的复发率分别为7.4%、0%和0%,5%咪喹莫特乳膏的局部不良反应发生率为79.2%,主要表现为用药局部红斑(58.3%)、瘙痒(41.7%)、水肿(37.5%).灼热(33.3%).疼痛(29.2%)和糜烂(25.0%),其局部不良反应容易耐受而且暂停用药后可迅速痊愈.无系统性不良反应发生。结论:激光治疗后继用5%咪喹莫特乳膏8周,可能是一种安全而且可降低尖锐湿疣复发率的治疗方法。

Preliminary study on the decrease in recurrence of condyloma acuminatum after the combined treatment of laser therapy and 5 % imiquimod cream

Abstract:Objective: To determine the efficacy and safety of 5% imiquimod cream applied topically for 8 weeks after laser therapy in the treatment of condyloma acuminatum (CA). Methods: A prospective study of 64 patients with CA was conducted. Thirty-six patients were treated only by laser therapy (laser therapy group) and 28 were treated with 5% imiquimod cream for 8 weeks. The cream was applied when the wound was healing after the laser therapy (combined treatment group). The recurrence and side effects were observed at week 4, 8 and 12. Results: At week 4, 8 and 12, the recurrence rates in laser therapy group were 25.0%, 13.3% and 3.7% respectively, while the recurrence rates in combined treatment group were 7.4%, 0% and 0% respectively. The local side effect related to the 5% imiquimod cream was 79.2%, including erythema (58.3%), itching (41.7%), edema (37.5%), burning (33.3%), pain (29.2%) and erosion (25.0%). No systemic side effect was found. Usually the side effects were tolerated and rapidly resolved after stopping the treatment. Conclusion: Treatment with 5% imiquimod cream for 8 weeks after laser therapy appears to be a safety and efficacy therapeutic approach in reducing the recurrence of CA.
