胰腺癌术后肠内营养护理的应用

目的探讨术后早期肠内营养在胰腺癌患者中的应用效果及护理方法。方法将宜昌市中心人民医院2011年1月～2012年8月收治的40例胰腺癌患者，按照术后营养护理方式的不同分为观察组和对照组，每组各20例，两组基础护理方法相同，比较两组患者术后7d时的胃肠功能情况、营养学指标变化、免疫功能情况，并发症的发生率及术后3个月的生活质量。结果观察组术后7d的胃肠功能显著优于对照组，并发症显著少于对照组（P〈0．05）；营养支持7d时的血浆白蛋白、血浆前白蛋白和血红蛋白相比较，观察组明显优于对照组（P〈0．05）；营养支持7d的CD4＋、CD47CD8＋、IgA、IgG，观察组明显高于对照组（P〈0．05）；观察组术后3个月在躯体功能、心理功能、社会功能、物质生活状态方面的生活质量评分显著高于对照组（P〈0．05）。结论对胰腺癌患者术后早期采用肠内营养可改善患者的体液免疫情况和营养学指标，增加体抗力，减少并发症的发生。同时患者营养素平衡全面，安全价廉，合理方便，有利于提高术后生活质量。

Application on enteral nutrition care after pancreatic cancer postoperative

Objective To discuss the application on effect and nursing methods of the early postoperative intestinal used in pancreatic cancer. Methods 40 patients with pancreatic cancer in the Central People＇s Hospital of Yichang City from January 2011 to August 2012 were collected and divided into observation group and control group according to the different way of postoperative nutritional care, each group had 20 cases, two groups had the same basic nursing method, gastrointestinal function, nutrition index change, immune function of postoperative 7 d, the incidence of com- plications and postoperative 3 months of the quality of life of two groups were compared. Results Gastrointestinal function of 7 d after the operation in observation group was significantly better than that in control group, complica- tions less than that in control group significantly （P 〈 0.05）; Observation group in nutrition support 7 d plasma albu- min, blood plasma prealbumin and hemoglobin were significantly superior to control group （P 〈 0.05）; and CD4＋, CD4~ /CD8＋, IgA, IgG in nutrition support 7 d of observation group were significantly higher than those in the control group （P 〈 0.05）; The observation group 3 months after the operation in physical function, psychological function, social function, material life state of the quality of life score significantly higher than those in the control group （P 〈 0.05）. Conclusion With early postoperative intestinal application in the pancreatic cancer could be improve the humoral im- munity and nutrition index, increase the body resistance and reduce complications. At the same time, patients nutrient balance comprehensive, safety and low price, reasonable convenient, could be to improve the life quality.
