心脏手术中的高乳酸血症

手术刺激可引起不同程度的应激反应,导致糖代谢紊乱,对患者的预后和手术的恢复产生不良的影响。心脏手术围术期高乳酸血症的发生多倾向于老年、女性、低射血分数、低血色素、急诊、二次、复杂手术的患者。目前,乳酸酸中毒尚无特效治疗方法,治疗关键是针对病因进行治疗。再者是对症支持治疗和应用缓冲剂纠正酸中毒,常用缓冲剂有碳酸氢钠、胰岛素、二氯乙酸钠等。

Lactic Acidosis in Cardiac Surgery

Surgical stimulation can cause varying degrees of stress response,resulting in glucose metabolism disorder,and adversely affect the prognosis of patients and surgical recovery. Lactate concentration reflects the level of glucose metabolism,peripheral circulation and tissue blood oxygenation as,an indirect indicator. Understanding of lactic acidosis and its relationship with CPB will help the prevention and treatment of lactic acidosis and decrease its mortality significantly. This article reviews the latest developments in these areas.
