股骨颈骨折手术空心钉内固定的生物力学综合分析

摘    要:目的 复位和三枚空心钉内固定是目前治疗股骨颈骨折的常用方法,利用有限元方法对空心钉内固定手术进行力学综合分析,可以找出一种符合生物力学规律的内固定治疗方法.方法 采用材料力学理论计算,分析了人单足站立时,骨折面所承受的螺钉预紧力和髋关节作用力所产生的轴向压、垂直分力和由此引起的弯距对骨折面与螺钉的综合力学效果.结果 获得了三颗空心钉内固定手术后,由各种力的综合作用所产生的螺钉及骨折面产生的内应力,并与许用应力值进行了比较.结论 从力学角度看,用空心钉内固定治疗股骨颈骨折手术是安全可靠的,术后病人可以快速复原,而且三枚空心钉的倒三角布局力学效果优于正三角布局.

Abstract:Objective Close reduction and internal fixation with 3 cannulated screws is a widely used method in treating femoral neck fractures.Using the Finite Element Method,postoperative biomechanical parameters can be analyzed comprehensive analysis the operation parameters,and optimal parameters can be found according to the theory of biomechanics.Methods Using the method of material mechanics,the pretightening load on cannulated screw and the comprehensive mechanics effect on fracture plane from hip joint were analyzed when people stand with one leg.Results The laws of stress distribution in the cannulated screws and fracture plane of the femoral neck were calculated,and compared with the permissible stress.Conclusion The internal-fixation with cannulated screws for the femoral neck fracture can make the fixation firmly,fast the healing of fracture,the treating effect is satisfied.And the best screw distribution is an upside down triangle.
