IgG型抗内皮细胞抗体在神经精神狼疮发病中的作用

目的探讨IgG型抗内皮细胞抗体（IgG-AECA）在神经精神狼疮（NPSLE）发生、发展中的作用及机制。方法采用免疫荧光法检测102例系统性红斑狼疮（SLE）患者（活动期62例，其中NPSLE 27例，非NPSLF35例，缓解期40例）的Ig,G-AECA。结果IgG-AECA的阳性率在活动期、缓解期患者分别为56．5％、22．5％（P〈0．05）。NPSLE者阳性率和滴度明显高于非NPSLE者（P〈0．05）。NPSLE患者治疗前明显高于治疗后（P〈0．05）。结论IgG-AECA在SLE发生中起重要作用，与病情活动密切相关；IgG-AECA可作为评估SLE系统损害、监测疾病活动性、制定合理治疗方案的指标。

Role of IgG -antiendothelium cell antibodies in the pathogenesis of neuropsychiatric systemic lupus

Objective To investigate the role of IgG -antiendothelial cell antibody （IgG-AECA）in the occurrence and development of neuropsychiatfic systemic lupus （NPSLE） and its mechanism. Methods One hundred and two patients with SLE were enrolled in this study ,62 cases （include 27 NPSLE and 35 non- NPSLE）of which were in active stage , and 40 cases were in remission stage. The serum level of IgG-AECA were detected by indirect immunofluorescence （IIF） technique. Results The IgG-AECA positive rate of patients in active stage and remission stage were respectively 56.5% and 22.5% （ P 〈 0.05 ）. The positive rate and titer of IgG-AECA in the NPSLE patients were significantly higher that those in the non- NPSLE patients （ P 〈 0.05 ）. The titer of IgG-AECA in the NPSLE patients decreased after the treatment （ P 〈 0.05）. Conclusions IgG-AECA may play an important role in the occurrence of SLE, and be closely related to the disease activity. IgG-AECA can be used as a serum marker for evaluating organ lesions, adjusting disease activity and deciding rational therapy plan.
