原发性食管小细胞癌的诊断治疗

目的对原发性食管小细胞癌的病理学特征、诊断治疗等相关问题进行分析.方法对我院1998年12月～2003年10月手术治疗的7例食管小细胞癌患者的临床病理资料进行分析.结果食管小细胞癌占同期食管癌手术例数的0.62%,术前确诊率低.全组病例手术切除率100%,无手术死亡.经随访,肿瘤无局部复发,但多因肿瘤血循环转移而死亡.目前仅1例生存期超过3年.结论原发性小细胞癌是一种临床上较为少见,但恶性程度很高的肿瘤,预后差;治疗上要早期进行规范的根治性切除术,术后就给予系统的辅助化疗以改善其预后.

Diagnosis and Treatment of Primary Small Cell Carcinoma of the Esophagus and a Review of Literatures

Objective To analyze the pathological feature, diagnosis and treatment of primary small cell carcinoma of the esophagus. Methods The clinical and pathological date of 7 patients underwent operation for primary small cell carcinoma of esophagus in our hospital during December,1998 to October,2003 was retrospectively analyzed. Results The patients with primary small cell carcinoma of esophagus accounted for 0.62% of the patients with esophageal carcinoma in the same period. The rate of patients definitely diagnosed before operation was lower. The overall resection rate was 100%.There was no death caused by operation. No one had local recurrence but most of them died of extensive metastasis during follow-up. So far only one patient survived over three years. Conclusion Primary small cell carcinoma of the esophagus is a rare but very malignant tumor. Its prognosis is very poor. In order to improve its prognosis , primary small cell carcinoma of the esophagus should be radically removed as soon as possible, followed by systematic chemotherapy after operation.
