经阴道彩色多普勒超声对清宫术后绒毛植入的诊断价值分析

目的探讨清宫术后及剖宫产术后瘢痕绒毛植入的超声特征.方法对5例清宫术后及剖宫产术后瘢痕绒毛植入患者经阴道彩色多普勒超声检查,分析其二维及彩色多普勒图像,对实验室结果、治疗进行随访.结果 2例有多次清宫术史,3例有剖宫产史,再次妊娠行清宫术后阴道流血,血尿HCG持续阳性.超声检查2例发现子宫内紧邻内膜线的增强不均质回声包块,考虑为宫腔组织残留而再次清宫,未见残留组织,经治疗后包块缩小,血尿HCG降至正常;3例发现子宫前壁下段不均质包块.结论经阴道彩色多普勒超声对绒毛植入有诊断意义.

Transvaginal Color Doppler Ultrasound in Diagnosis of Chorionic Villi Implantation after Curettage

Objective To study the ultrasonic characteristics of chorionic villi implantation after curettage and cesarean section.Methods Five patients with chorionic villi implantation after curettage and cesarean section were given transvaginal color Doppler ultrasound examination,and the two-dimensional and color Doppler imaging was analyzed.All patients were followed up and the laboratory results and treatment was observed.Results Two patients had a history of repeated curettage,and three patients had a history of Cesarean section,vaginal bleeding after curettage in pregnancy again,sustained positive urine HCG.After ultrasound examination,2 cases were found with enhanced heterogeneous mass in the part close to the uterus endometrium line,uterine tissue residues was considered and the patients were given curettage again,but no uterine tissue residues were found.The masses shrinked and HCG levels in serum and urine were decreased to normal levels after treatment.3 cases were found with heterogeneous mass in the anterior uterine wall.Conclusion Transvaginal color Doppler ultrasound is valuable in the diagnosis of chorionic villi implantation.
