颌骨大型牙源性囊性病变的微创治疗

目的探讨袋成形术联合应用阻塞器治疗颌骨大型牙源性囊性病变的临床效果。方法颌骨大型牙源性囊性病变患者36例,采用袋成形术联合应用阻塞器治疗,术后3、6、12个月复诊,行临床检查及拍摄X线曲面断层片,记录囊腔影像面积变化,术后6~12个月对仍存留的囊肿行Ⅱ期囊肿刮切术。结果 36例患者术后3~6个月面部膨隆畸形均基本消失或完全消失,无1例患者出现伤口感染;10例患者囊腔体积缩小大于90%,24例囊腔体积缩小率达到80%~90%,2例囊腔体积缩小在60%以上;Ⅱ期囊肿刮切术后继续随访1年囊肿无复发,形态功能均恢复良好。结论袋成形术联合应用阻塞器治疗颌骨大型囊性病变,可改善患者颌骨膨隆畸形,最大程度保留颌骨解剖结构及生理功能,疗效肯定。

Observation of effect of minimally invasive treatment for large maxillary odontogenic cysts

Objective To explore the minimally invasive treatment effect of blocking device combined with bag keratoplasty on large maxillary odontogenic cysts. Methods Thirty-six patients were collected from January 2006 to August 2013 treated by oral and maxillafacial surgery with large maxillary odontogenic cystic lesion, combined using of bags of keratoplasty and blocking device were adopted fnr the treatment, patients were followed up for 3, 6, 12 months for clinical examination and X-ray examination, changes of the lumen of the surface were recorded. After scraping surgery of cyst in stage Ⅱ , follow-up continued 6 to 12 months. Results Three to six months after surgery, thirty-six patients facial deformities completely disappeared, anti no wound infection occurred in one patient; cavity volume reduction of 10 case were more than 90% , the cyst disappeared ; cysts volume reduction of 24 cases were 80% - 90% ; volume cavity of 2 cases reduced more than 60% ; after finally cyst curettage were conducted, follow-up continued 1 year after the cyst without recurrence, morphology and function recovered well. Conclusion Combined use of bag of keratoplasty and blocking device for large jaw cystic lesion have significantly reduce and eliminate effect, improved jaw bone deformity, retained the anatomical structure and physiological function.
