剖宫产术后应用硬膜外自控镇痛的效果观察

①目的　探讨剖宫产术后应用硬膜外自控镇痛(Epiduralpatientcontrolledagalgesia,EPCA)的效果。②方法　对300例剖 宫产孕妇术后应用EPCA,并设立随机对照组,观察镇痛效果以及对母乳喂养、子宫复旧、产妇生命体征的影响。③结果　使用EPCA, 镇痛有效率为98.06%,镇痛组产妇产后24小时内开始泌乳例数明显多于对照组(P>0.05),差异有显著性。产妇的心率、呼吸、血 压、血氧饱和度与对照组相比,差异无显著性。两组产后出血量相比差异无显著性。④结论　使用EPCA可取得良好的镇痛效果,并 能提高母乳喂养的成功率,但不影响子宫肌层的缩复和产妇的生命体征。

Observation of the effect of epidural patient controlled agalgesia on cases after caesarean operatn

Objective To investigate the effect of Epidural patient controlled agalgesia(EPCA) on cases after caesarean operation.Methods 300 pregnant women after caesarean operation applied the EPCA,while randomized controlled cases group was established.The analgesic effect and other indices,such as the influences on feeding the baby on mother's milk,in volution of uterus,and vital sign were observed.Results The valid rate of the analgesia by EPCA was 98.06%.In the analgesia group,the cases of the puerperas'milk secretion in 24 hours of postpartum were far more than those in the conrtol group(P<0.05).There were no significant differences in the puerpera's vital sign,e.g.heart rate,respiration,blood pressure,blood oxygen saturation degree and postpartum bleeding amount between the analgesic group and control group.Conclusion The application of EPCA gets the good analgesic effect and raises success rate of feeding baby on mother's milk,while doesn't interfere with involution of myometrium and puerpera's vital sign.
