非小细胞肺癌患者血清MMP-2 TIMP-2水平及鸦胆子乳剂的调控作用

目的观察非小细胞肺癌患者血清MMP-2、TIMP-2水平及鸦胆子乳剂的调控作用.方法随机将40例非小细胞肺癌患者分成对照组及治疗组各20例.治疗组鸦胆子乳剂注射液30 mL qd×30天+TP方案静脉化疗2疗程.对照组单用TP方案(用法剂量同治疗组)静脉化疗2个疗程.分别于化疗前1天、第2个疗程结束后(即第42天)按酶联免疫吸附测定(enzyme-linked immunosorbent assay简称ELISA)技术要求收集标本送检.结果血清MMP-2、TIMP-2水平与TNM分期呈线性关系,MMP-2水平随TNM分期增高而升高,TIMP-2则正好相反.治疗组其下调MMP-2及上调TIMP-2作用较对照组显著,P＜0.05;两组有效率无显著性差异(P＞0.05);治疗组对化疗所致的Ⅲ-Ⅳ度WBC损伤具保护作用(x2=3.956,P＜0.05);两组生存质量差异显著(P＜0.05).结论非小细胞肺癌患者血清MMP-2、TIMP-2水平可作为判断肺癌预后指标之一.鸦胆子乳剂具下调非小细胞肺癌患者血清MMP-2及上调TIMP-2的作用,即有一定的MMP抑制物样作用.

The MMP -2 and TIMP -2 Levels in Blood Serum of Patients with NSCLS in Different TNM Staging and Regulation of YaDanZiRuJi

Objective: To investigate the MMP-2 and TIMP-2 levels in blood serum of patients with NSCLC in different TNM staging and regulation of YaDanZiRuJi on them.Methods:40 cases of NSCLC were randomizedly divided into treatment group and control group.Patients in treatment group were treated by YaDanZiRuJi 30mL/day on d1-30 and chemical treatment(CT) with TP regimen for two cycles. Patients in control group were treated by CT with TP regimen for two cycles,every 21 days were as one cycle for CT.Blood serum of every case was collected on d0 and d42 respectively.Results:The MMP-2 and TIMP-2 levels in blood serum of all cases were linear relationship with TNM staging.The MMP-2 levels were positively associated with TNM staging,the TIMP-2 levels were negatively associated with TNM staging. The MMP-2 levels in blood serum of cases in treatment group were lower,and the TIMP-2 levels were higher,there were significant differences(P<0.05);The prevention of G_ 3 -G_ 4 WBC toxicity and the improvement of quality of life(QOL) in two groups were significant differences (P<0.05);Meanwhile,the beneficial rates were no significant differences.Conclusion:The MMP-2 and TIMP-2 levels in blood serum of patients with NSCLC would be one of prognostic signs and YaDanZiRuJi shows a kind of effect of TIMP.Ongoing clinical trial would be considered.
