带状疱疹后遗神经痛患者的感觉定量测定

目的：研究不同临床表现的带状疱疹后遗神经痛（PHN）的发病机制。方法：用感觉定量分析仪对25例PHN患者的疼痛区及对侧镜象区进行定量感觉测试，并用视觉类比量表评价疼痛的强度。结果：自发性疼痛组（8例）感觉缺失值大于触觉异常性疼痛组（17例）；在触觉异常性疼痛组，感觉缺失程度与疼痛评分成负相关，而在自发性疼痛组，二者间未见显著相关性。结论：触觉异常性疼痛与感觉传入小纤维的活性增高、异常放电有关，而自发性疼痛与脊髓神经元的自发性痫样放电有关。

Quantitative sensory testing in patients with sequent neuralgia of herpes zoster

Objective: To study the mechanisms of postherpetic neuralgia(PHN) in patients with different symptoms.Methods: The sensory quantitative test(QST) was performed on both painful skins and contralateral mirror-image areas in 25 patients with PHN by Quantitative sensory analyzer,and at the same time the pain intensity were measured by visual analogue scale(VAS).Results:The scores of sensory deficits in PHN patients with spontaneous pain(8 cases)were more than those in patients with allodynia(17 cases).A significant correlation between the degree of sensory deficit and pain intensity(the score by VAS) was noted in the PHN patients of allodynia,but there was not significant correlation between them in PHN patients with spontaneous pain.Conclusion:The allodynia might be related to the abnormal discharges and increased activity of the sensory afferent fibres,while the spontaneous pain might be related to the epileptiform discharges of the spinal cord neurone.
