非酒精性脂肪肝患者脂质过氧化与肝功能的相关性研究

摘    要:目的探讨非酒精性脂肪肝(NAFLD)患者血清脂质过氧化物与肝功能的相关关系,为阐明NAFLD的发病机制提供依据。方法采用配对病例-对照研究,检测了150例NAFLD患者及150例健康对照者的血清丙二醛(MDA)、超氧化物歧化酶(SOD)、丙氨酸转氨酶(ALT)、天冬氨酸转氨酶(AST)四个指标,比较上述指标差异并进行Pearson相关分析。结果 NAFLD组患者血清MDA、ALT、AST分别为(2.61±0.90)nmol/mL、(38.34±23.31)U/L、(32.66±12.36)U/L,高于对照组(2.42±0.93)nmol/mL、(23.53±12.74)U/L、(29.55±9.53)U/L(P<0.05),SOD活性为(75.77±15.97)U/mL,低于对照组(80.65±15.20)U/mL(P<0.01)。相关分析发现全部研究对象的MDA与ALT呈正相关(r=0.125,P<0.05),SOD活性与AST呈正相关(r=0.191,P<0.01);病例组的SOD与AST呈正相关(r=0.241,P<0.01)。结论 NAFLD患者血清脂质过氧化水平增高,并与肝功能之间有一定的关联。

Abstract:Objective To investigate the relationship between serum lipid peroxidation and liver function levels in the patients with non-alcoholic fatty liver(NAFLD),so as to provide evidence for illuminating the pathogenesis of the disease.Methods Levels of serum malomdialdehyde(MDA),superoxide dismutase(SOD),alanine aminotransferase(ALT),aspartate aminotransferase(AST) were measured and compared between 150 patients and 150 healthy subjects by a matched case-control study,and Pearson correlation analysis was employed to explore the relationships between these indexes.Results The levels of serum MDA,ALT,AST in NAFLD patients(2.61±0.90 nmol/ml,38.34±23.31 u/L,and 32.66±12.36 u/L,respectively) were higher than those in healthy controls(2.42±0.93 nmol/ml,23.53±12.74 u/L,29.55±9.53 u/L)(P<0.05),but level of serum SOD(75.77±15.97) U/ml was lower in NAFLD group(80.65±15.20) U/ml(P<0.01).Pearson correlation analysis revealed that ALT was significantly correlated with MDA(r=0.125,P<0.05),and AST with SOD(r=0.191,P<0.01) in all subjects.In the patients with NAFLD,SOD was significantly correlated with AST(r=0.241,P<0.01).Conclusion Serum level of lipid peroxidation is increased in the patients with NAFLD,and correlated with liver function to a certain extent.
