医源性泌尿系损伤28例原因分析

目的 探讨医源性泌尿系损伤的原因。方法 对1982年至2002年28例医源性泌尿系统损伤的病例进行回顾性分析。结果 妇科、泌尿外伤、普外科手术所造成的泌尿系损伤分别为：妇科22例(占79％)，泌尿外伤5例(占18％)，普外科1例(占4％)，以妇科所引起的发病率最高。其中膀胱损伤的10例，妇科节育术8例，子宫切除术2例。输尿管损伤的16例中妇科手术占12例，泌尿科手术2例，普外科手术1例。尿道假道形成2例。均泌尿科手术所致。结论 熟练掌握局部解剖关系，仔细解剖分离组织是预防医源性泌尿系统损伤的关键。

The cause analysis of iatrogenic urinary system injuries(Report of 28 cases)

Objective to discuss the cause of iatrogenic urinary system injuries. Methods Together with review of literatures, 28 cases of iatrogenic urinary systeminjuries were reported and the epidemiology, clinical manifestations between 1982 and 2002. Results urological, gynecological and general surgical procedures accounted for 5(18%), 22(79%), 1(4%) injuries. The incidence of gynecological injuries is most. Incluse 10 cases of trauma of bladder, 8 cases of gynecological birth control, 2 cases of uterus excision. All thr 16 cases of ureteral injuries include 12 cases of gynecological operation,2cases of uroloigical operation, 1 cases of general surgical operation. 2 cases of trauma of urinary tract all because urological operation. Conclusions It was suggested that careful and exact oprration as well as acquaintance with the anatomy of urinary system before and during surgery be key to precaution of iatrogenic urinary injuies.
