卡维地洛与卡托普利防治急性心肌梗死后左心室重构的对比研究

目的比较卡维地洛与卡托普利对AMI后左心室重构的效果。方法将122例AMI患者随机分为卡维地洛组52例,卡托普利组50例,常规治疗组20例,分别于治疗后1周、6个月采用心脏多普勒超声测定左心室间隔厚度、左室后壁厚度、左室舒张末内径,计算左室重量指数,同时测定左室射血分数及舒张早期左室充盈最大速率(E峰)与心房收缩期最大速率(A峰)。结果卡维地洛组和卡托普利组与常规治疗组比较均可使左心室间隔厚度、左室后壁厚度变薄(P<0.01),LVDd缩小(P<0.01),左室重量指数降低(P<0.01)。卡维地洛组和卡托普利组均可增加LVEF、降低A/E峰比值,与常规治疗组比较,1周时P<0.05,6个月时P<0.01。结论卡维地洛具有良好的防治AMI后左室重构效果。

Comparision of the Effect of Captopril and Carvedilol Which was Used to Prevent and Cure Reconstruction of Left Ventricle After Acute Myocardial Infarction

Objective The aim of this study is to compare the effect of captopril and carvedilol which was used to prevent and cure reconstruction of left ventricle afert AMI. Methods 122 patients were randomly divided into three groups: the first cure group of carvedilol (52 patients ), the second cure group of captopril (50 patients ) ,the third cure group of common (20 patients ). After one weeks and six months, we determined left interventricular septum thickness(IVST), the left ventricular past-wall thickness(LVPWT), the left ventricular and diastolic diamater (LVDd) ;we counted left ventricular weight index (LVMI).Meanwhile , we determined the letf ventricular ejection figure (LVEF) . The fastest velocity of left ventricular enjection (E peak) in the early stage of diastole and in the period of left cotrium systolic(A peak). Results Carvedilol and Captopril were associated with significant curative effect on reducing IVST, LVPWT, LVDd, LVMI(P<0.01) in comparing with the common cure group. Both groups could add LVEF and reduce A/E in comparing with the common group, ( after one weeks ,P<0.05; after six monthes P<0.01).Conclusions Carvedilol has good effect on prevention and cure left ventricular after acute myocardial infarction .
