双心室起搏再同步治疗10例慢性充血性心力衰竭患者近期疗效

目的:观察双心室起搏再同步化治疗（CRT）慢性充血性心力衰竭（CHF）患者的临床疗效。方法: 选取经药物治疗但逐步加重的CHF患者10（男6，女4 ）例，年龄（62±10）岁，心功能（NYHA）Ⅲ～Ⅳ级，全部置入双心室三腔起搏器（9例InSync Ⅲ 8042型，1例FRONTIER Ⅱ 5596型），左室电极置于冠状静脉侧支或后侧支分支内。观察CRT前后左心室功能及体表心电图的变化。结果: 10例患者均顺利完成起搏器置入。起搏方式均为心房感知心室起搏，其中总心室起搏数>90%。所有患者的临床指标明显改善，心功能(NYHA)分级、6分钟步行试验、左室射血分数、左室舒末内径、生活质量评分等较术前显著提高(P<0.05,P<0.01)。结论: CRT治疗CHF患者可改善患者心功能，提高生活质量。

Clinical effect of biventricular pacing on patients with congestive heart failure: report of ten cases

AIM:To observe the clinical effect of biventricular pacing in patients with congestive heart failure (CHF). METHODS: Three-chamber biventricular pacemakers (nine InSync III 8042 and one FRONTIER II 5596) were installed in ten patients (six males and four females) with a mean age of (62±10) years with repeated CHF who had not responded well to medication. Heart functions and ECG were observed before and after the installation. RESULTS: Three-chamber biventricular pacemakers were successfully installed in all ten patients. The pacing mode was VAT and the total percentage of ventricular pacing was >90%. Significant clinical improvements were achieved as evidenced by better results of heart functions, 6-min walk, left ventricular ejection fraction and left ventricular end-diastolic diameter (P<0.05, P<0.01). CONCLUSION: Biventricular pacing can improve cardiac functions and enhance the quality of life in patients with congestive heart failure.
