低分子肝素和普通肝素在主动脉内球囊反搏术中抗凝应用的临床观察

摘    要:目的:观察低分子肝素(LMWH) 和普通肝素(UFH)在主动脉内球囊反搏术(IABP)中抗凝应用的临床效果。方法: 68例安置IABP的患者随机分为应用UFH抗凝的常规组(34例)和应用LMWH抗凝的试药组(34例),观察两组患者的出血、血肿、血栓形成等并发症发生情况及费用差别。结果: 两组均只有少部分患者有伤口少量渗血、局部小血肿和伤口周围轻度青紫等表现,无其它并发症出现,两组比较差异无显著性。但试药组的费用比常规组低,两组比较差异有显著性。结论: 在IABP中应用LMWH可以达到应用UFH相同的抗凝、预防血栓形成的效果,同时并发症无增加,而费用减少。

Abstract:AIM:To observe the clinical effect of low molecular weight heparin (LMWH) compared with unfractionated heparin (UFH) in intra-aortic balloon pump (IABP) surgery. METHODS: Sixty-eight patients with IABP were randomly divided into two groups: conventional group (34 cases) using UFH anticoagulation and observation group (34 cases) using LMWH anticoagulation. The incidence of bleeding, hematoma, thrombosis and other complications, as well as the cost in the two groups, were recorded. RESULTS: Only slight wound bleeding, local small hematoma and mild bruising around the wound were observed in a low percentage of the patients, with no significant difference between groups. However, the cost in the observation group was significantly lower than in the conventional group. CONCLUSION: LMWH has a low cost and is more convenient and if as effective as UFH in regard to anticoagulant and antithrombotic effect for IABP.
