儿童耳鼻咽喉原发恶性肿瘤(附70例)

目的探讨儿童耳鼻咽喉原发恶性肿瘤的特点，提高临床诊治水平。方法回顾分析1965～1999年间我院收治的70例14岁以下儿童耳鼻咽喉原发恶性肿瘤的临床资料。结果非上皮源性恶性肿瘤56例，占总数的80％，包括横纹肌肉瘤19例，纤维肉瘤8例，软骨肉瘤4例，黑色素瘤12例，嗅神经母细胞瘤4例，恶性淋巴瘤9例；上皮来源肿瘤14例，占总数的20％，包括未分化癌9例，乳头状癌5例；治疗采用手术切除 放疗和／或化疗37例，单纯手术切除4例，非手术治疗(放疗和／或化疗)20例，放弃治疗9例。获随访资料61例，存活3年以上者11例，均为接受手术切除 放疗或化疗者，3年生存率为18．03％。结论儿童耳鼻咽喉原发恶性肿瘤恶性度高，预后差，尽早确诊并采用综合治疗可望取得较好疗效。

The clinical analysis of malignant tumors of otorhinolaryngologic region in children

Objective To explore the clinical characte ri stics of malignant tumors of otorhinolaryngologic region in children and to impr ove the level of diagnosis and therapy. Methodes A total 70 cases of malignant tumors of otorhinolaryngologic region in children between 1965 and 1999 were analyzed retrospectively.Results Of t he cancers,nonepithelail malignant tumor accounted 80%,epithelail malignant tum or 20%.Treatment included surgery, irradiation or/and chemotherapy and surgery plus irradiation or chemotherapy . Sixty one cases were followed up,11 cases sur vived over 3 years,and they all accept the treatment of surgery plus irradiation or chemotherapy.The survival rate of 3 years was 18.03%.Conclusion Malignant tumors of otorhinolaryngologic region have aggressive behav ior and poor prognosis. The keys of efficient treatment and preventing from recu rrence are earlier diagnosis and selecting comprehensive therapy.
