经腹腔镜子宫动脉阻断术加肌瘤剔除术治疗子宫肌瘤215例临床疗效观察

目的 观察经腹腔镜子宫动脉阻断术加肌瘤剔除术治疗子宫肌瘤的临床疗效.方法 回顾性分析上海市杨浦区中心医院2007年1月至2008年12月482例子宫肌瘤手术患者的临床资料,分为治疗组(经腹腔镜子宫动脉阻断术加肌瘤剔除术)215例和对照组(经腹腔镜子宫肌瘤剔除术)267例,记录两组患者手术时间、术中出血量、术后排气时间、离床活动时间、术后住院时间、术后并发症、术后2年随访复发率等,并进行统计学处理.结果 两组患者在手术时间、术后排气时间、离床活动时间、住院时间方面比较,差异无统计学意义(P＞0.05)；在术中出血量、术后并发症、复发率方面比较,差异有统计学意义(P＜0.05).结论 经腹腔镜子宫动脉阻断术加肌瘤剔除术既具有腹腔镜手术损伤小、恢复快等优点,又能减少术中出血、减轻组织损伤、降低术后复发率,具有临床推广应用价值.

Clinical effective observation on laparoscopic uterine artery blockage and myomectomy for treating hysteromyoma in 215 cases

Objective To observe the clinical effect of laparoscopic uterine artery blockage and myomectomy for treating hysteromyoma.Methods The clinical data of 482 cases with hysteromyoma in Yangpu District Central Hospital from January 2007 to December 2008 were selected and analyzed retrospectively.All cases were divided into the treatment group（215 cases,laparoscopic uterine artery blockage and myomectomy）and the control group（267 cases,laparoscopic myomectomy）.The operation time,intraoperative bleeding quantity,postoperative exhaust time,ambulation time,postoperative hospital stay,postoperative complications and postoperative 2-year follow up recurrence rate were recorded and performed the statistical process.Results The operation time,postoperative exhaust time,ambulation time and hospital stay had no statistical difference between the two groups（P〈0.05）.Intraoperative bleeding quantity,postoperative complications and recurrence rate had statistical difference between the two groups（P〈0.05）.Conclusion Laparoscopic uterine artery blockage and myomectomy not only have the advantages of little damage,quick recovery,etc.in laparoscopic operation,but also can reduce the intraoperative bleeding,alleviate the tissue damage,decrease the postoperative recurrence rate,which has the clinical application and promotion value.
