64层螺旋CT最小密度投影结合CT仿真内窥镜对小儿支气管异物的应用价值

摘    要:目的 探讨64层螺旋CT最小密度投影(MinIP)结合CT仿真内窥镜(CTVE)技术对小儿支气管异物的应用价值.方法 对72例临床拟诊为支气管异物患儿行64层螺旋CT检查,分析64层螺旋CT MinIP结合CTVE等多种重建技术对小儿支气管异物显示情况,并与纤维支气管镜检查结果对照.结果 72例患儿中64层螺旋CT MinIP结合CTVE技术诊断气管支气管异物42例,以纤维支气管镜为标准,敏感性为94.6%,特异性为95.4%,诊断准确率为93.2%;两种方法对支气管异物的检出率比较,差异无统计学意义(X2=0.174,P=0.676).结论 64层螺旋CT MinIP结合CTVE技术是一种快速、无创的检出方法,大大提高了检查小儿支气管异物的敏感性、特异性和栓出率,对小儿支气管异物纤维支气管镜取出治疗有重要参考价值.

Abstract:Objective To expore the the application value of minimum intensity projection(MinIP) of 64 slice spiral CT(64-SSCT) combined computed tomographic virtual endoscopy(CTVE) in children tracheobronchial foreign bodies.Methods 72 children patients with clinically suspected tracheobronchial foreign bodies underwent 64-SSCT and bronchoscope.The displaying findings of MinIP of 64-SSCT combined CTVE in children tracheobronchial foreign bodies were analyzed and compared with the bronchoscopic results.Results Of all the 72 children cases,42 cases were found tracheobronchial foreign bodies by MinIP of 64-SSCT combined CTVE.With the bronchoscopy as the standard,the sensibility,specificity and diagnostic accuracy rate of MinIP combined CTVE were 94.6%,95.4% and 93.2%,respectively.The difference in the detection rate between MinIP combined CTVE and bronchoscopy had no statistical significance(χ2=0.174,P=0.6760.05).Conclusion MinIP of 64 slice spiral CT combined CTVE is a quick and noninvasive examination method,greatly increases the sensibility,specificity and detection rate in children tracheobronchial foreign bodies and has an important value for the foreign body extraction treatment of bronchoscope.
