关节腔内应用布托啡诺对膝关节镜术后镇痛的疗效观察

【目的】观察全身麻醉下行关节镜手术患者关节腔内应用小剂量布托啡诺对术后镇痛的效果。【方法]选择50例择期全身麻醉下行关节镜手术病人，随机分成A、B两纽，两纽病人性别、年龄、体重、手术时间相比较均无统计学差异。A组关节腔内注射布托啡诺0．01mg／kg（布托啡诺组），B组关节腔内注射同量生理盐水（安慰剂组）。予视觉模拟评分（VAS）、首次要求术后镇痛的时间对术后镇痛进行评价，由对实验不知情的人员进行观察与评分。观察至术后24h。【结果】与生理盐水组相比，关节腔内注射布托啡诺组的vAs评分明显降低，两组首次要求使用镇痛药物的时间A组明显长于生理盐水纽，且两组相比较差异有显著性（均P〈O．05）。【结论】与安慰剂相比，布托啡诺应用于关节腔内能够产生术后镇痛效果，但持续时间较短。

Randomized Double-blind Control Study of Intra-articular Butorphanol for Postoperative Analgesia of Knee Arthroscopy

Objective]To observe the effect of intrmarticular injection with low dose of butorphanol on postoperative analgesia of patients undergoing arthroscopy under general anesthesia. Methods] Fifty patients undergoing elec tire arthroscopy under general anesthesia were randomly divided into group A and B. There was no statistical differ ence in gender, age, body weight and operation time between two groups. Group A（butorphanol group） was injected with butorphanol 0.01mg / kg in articular cavity. Group B（placebo group） was injected equal saline in articular cavity. Visual analogue score and time of the first requirement for postoperative analgesia was used for evaluating the postoperative analgesia. Postoperative analgesia was observed and scored until 24 hours after operation. Results] Compared with normal saline group, visual analogue score in butorphanol group decreased obviously. The time of the first requirement for analgesics in group A was obviously longer than that in normal saline group, and there was significant difference between two groups（all P 〈0.05）. Conclusion] Compared with placebo group, intr〉articular bu torphanol has analgesic effect with shorter duration.
