特发性频发室性早搏患者经导管射频消融术后血浆BNP水平变化的临床研究

摘    要:【目的】探讨心功能正常的特发性频发室性早搏(FVC)患者经射频消融术(RFcA)后血浆脑钠肽(BNP)的变化及其意义。【方法]33例接受RFCA成功治疗的特发性频发PVC患者为室早组,总数为(16391.03±10873.01)个/天。另选53例健康体检者作为对照组,两组均检查心脏彩超、动态心电图及血浆BNP浓度,室早组术后3个月复查,分别比较术前、术后及对照组左室射血分数(LVEF)、左室舒张末内径(LVEDd)及血浆BNP浓度。【结果】对照组血浆BNP水平显著低于室早组术前水平(P〈0.01);室旱组患者手术后血浆BNP水平较手术前明显降低(P〈0.01);对照组与室早组术后相比血浆BNP水平无显著差异(P〉0.05);对照组与室早组术前相比、室旱组手术前后比较及对照组与室早组术后相比LVEF及LVEDd差异均不明显(P〉0.05)。【结论】心功能正常的特发性频发PVC患者血浆BNP水平升高,RFCA有效治疗PVC后可使患者血浆BNP降低,在心脏超声检查心脏结构改变不明显的情况下采用BNP来判断PVC患者病情严重程度、发展及预后情况有重要意义。

Clinical Study on the Change of Plasma BNP in Patients with Idiopathic Frequent Ventricular Premature Contraction after Radiofrequency Catheter Ablation

Abstract:ObjectivelTo explore the change of plasma B-type natriuretic peptide(BNP) in idiopathic frequent ventricular premature contraction patients with normal cardiac function after radiofrequency catheter ablation(RFCA) and its significance. Methods]Thirty-three patients with frequent ventricular premature contraction receiving RFCA successfully were selected as ventricular premature contraction group with a total of 16,391. 03+10,873. 01 beats per day. Other 53 healthy subjects were selected as the control group. Heart color echocardiography, dynamic electrocardiogram and plasma BNP in two groups were examined. The reex- amination of ventricular premature contraction group was taken 3 months after operation. Left ventrieular ejection fraction(LVEF), left ventricular end-diastolic dimension(LVEDd) and plasma BNP in were compared between two groups and before and after operation. Results]Plasma BNP in control group was significantly lower than that in ventrieular premature contraction group before operation( P 〈0.01). Compared with before operation, plasma BNP in ventrieular premature contraction group after obviously decreased after operation( P 〈0.01). There was no significant difference in plasma BNP between control group and ventricular premature contraction group after operation( P〉0.05). There was no obvious difference in LVEF and LVEDd among all groups( P 〉0.05). Conclusion]Plasma BNP in idiopathic frequent ventricular premature contraction patients with normal cardiac function is increased. RFCA can effectively treat the ventricular premature contraction and decrease the plasma BNP of patients. When the cardiac structure examined by heart color echocardiography has no obvious change, plasma BNP can be used to evaluate the severity, development and prognosis of patients with ventricular premature contraction.
